Care Assurance Community Care Services at Home
We help seniors and adults with functional and/or cognitive impairments obtain access to care that allows them to have a choice in the care that they desire. Clients can stay within their homes and receive services needed to support them with healthcare needs. Our case management services are available and serve as a long term partnership in care coordination.
What is Care Coordination?
Care coordination benefits the client and their circle of care in a person-centered care planning process. The Care Assurance care planning process is a multi-layered service development program in which we help our clients access the care they need and ensure satisfaction and appropriateness of care and services.
Programs Offered
Care Assurance Aging Solutions is an approved Georgia Department of Community Healthcare approved case management site for CCSP/SOURCE and ICWP waiver programs.

In addition to Medicaid related services, Care Assurance Aging Solutions offer private pay based aging life care management services which includes helping seniors locate services within their needs and budget that allows for safe aging in place. We also provide continuous support and service coordination to encourage the safest aging in place capabilities.
